Our Part-Time Case Managers are Salaried and have the flexibility to build their own schedules while working with a team of visit nurses to provide in-home care to clients throughout Minneapolis and St. Paul. $3000 Sign-on bonus.

Job Responsibilities:
- Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards, providing initial assessments, re-assessments, LPN and CNA Supervisory Visits, providing competencies in-home to staff, and creating plans of care and task lists electronically.
- Outside of in-home visits and work, we offer the ability to work in office or remotely from home. NO OASIS, electronic charting via our EMR (ABS MOBILE).
- Collaborate with multidisciplinary team members to optimize patient care and outcomes. We provide unskilled and skilled services alike to our clients, including PT and OT, and utilizing home health aides to provide companionship, transportation, light housekeeping and hands-on cares, always keeping person-centered care at top of mind.
- Educate patients and their families on health maintenance and disease prevention.
- Ensure accurate documentation and compliance with all regulatory guidelines.
- Help us ensure quality care at our client's fingertips by being a part of our nurse on-call rotation, only 1 week every 3 months. Helping answer care concern questions or follow up on incidents that might occur after-hours.
- Manage a case load of 40-45 Nursing Units (20-30 clients) in your 3-Day per week availability.
- Additional per visit work available for 'pick-up with additional pay' only if desired outside of 3-Day per week working days.
Requirements:
- Active RN license in the state of Minnesota.
- Valid Driver's License, Current Auto Insurance and Reliable Vehicle to Use for Work.
- Previous experience in a home health or geriatric setting is advantageous.
- Exceptional interpersonal and communication skills building trust with clients, their families, and your clinical team.
- Committed to delivering high-quality, patient-centered care.
- Willingness to commute throughout the Twin Cities. Most of your clients will stay within a 25-mile radius of each other.
- Willingness to answer on-call for 1-week every 11-12 weeks of time.
- Negative TB Test.
- Cleared NetStudy Fingerprints prior to start date.
