Seeking a Physical Therapist 2 -3 days per week.
BrightStar Care is looking for a PT that loves in-home care and keeping clients happy, healthy and safe in their own homes! We are a home health agency that provides comprehensive, person-centered cares where our clients need it most; in the comfort of their home. We are a proud partner with the Veteran's Administration and serve our Veterans in the Twin Cities and Surrounding communities. We are looking for a new member of our team, interested in clients in the St. Paul and North Metro communities within the Twin Cities.

Key Responsibilities:
- Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients.
- All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
- Improves or minimizes residual physical disabilities of the patient.
- Returns the individual to optimum and productive level within patient’s capabilities.
- Participates with all other home care personnel in patient care planning.
- Takes initial history and makes initial evaluation.
- Consults with physicians regarding patient’s progress
- Participates in quality assessment and performance improvement activities as assigned.
- Prepares clinical and progress notes.
- Helps develop the plan of care and revise, as necessary.
- Consults with family and agency personnel.
- Ability to do extensive bending, lifting and standing on a regular basis.
Requirements:
- A graduate of a PT Program recognized by the American Physical Therapy Association or other National accepted Council or Association
- An active license to practice Physical Therapy in MN
- A minimum of four (2) years’ experience as a Physical Therapist, home care and geriatric experience preferred
- Must clear criminal background check
- Must have current CPR certification
- Must have active driver's license and clear driving record with reliable vehicle for work
- Computer skills.
- Ability to exercise initiative and independent judgment
